Please note that this form is a bit weird. You may look at it and think 
that nothing on the form applies to you...they talk about being
blind etc. However is DOESN'T MATTER. 
 
All your doctor needs to know is that your condition will be long term
and they know how to fill it out so that you will get excepted.

They usually say it takes you longer to do everyday tasks etc.
